GET ENROLLED!
To start your child’s enrollment process please download all the documents below. It’s very important to properly fill out all these documents. Then, you can simply send them to admin@miniartfamily.com, or upload then in the parent’s corner form at the bottom of the page.
STEPS FOR ENROLLMENT PROCESS
DOWNLOAD FILES
Click on the button below to download the .ZIP file to your computer with all the enrollment forms.
FILL OUT
Once you download all the forms, fill them out. Please make sure to give all the correct and exact information, this is very important for your child´s enrollment process.
SCAN DOCUMENTS
Once you properly fill out all the forms, just scan them or take clear pictures with your smarthphone, be sure all the information is clear and can be read as well.
SEND OR UPLOAD
When you finish filling, scanning or taking pictures of the forms you can simply save them and upload them into the contact form below, or send them in PDF format to admin@miniartfamily.com
